3. Scope of Work
The selected agency must conduct a baseline behavioural and quality-of-life assessment for NCD patients enrolled in the Lives Program.
Objectives
  1. Document behavioural patterns: diet, physical activity, tobacco/alcohol use, medication adherence.
  2. Compute behavioural risk scores using standardised tools.
  3. Measure quality of life using SF-12.
  4. Analyse behavioural risks & QoL variations by demographic groups.
  5. Classify patients by behavioural & QoL risk segments for counselling.
  6. Develop behavioural + well-being profiles for baseline vs. endline comparison.
Methodology

    2. Palghar: Jawahar, Wada
